50 vials of Moderna vaccine thrown out at Grafton clinic due to 'human error,' Advocate Aurora says

Dozens of vials of the Moderna COVID-19 vaccine had to be thrown out due to "human error," officials with Aurora Health Care said.

Advocate Aurora Health officials said the incident happened over the weekend, when an employee removed the vials from the Grafton clinic refrigerator in order to access something else and did not return them.

"We learned that about 50 vials of Moderna vaccine were inadvertently removed from a pharmacy refrigerator overnight," a spokesperson said. "While some of the vaccine was administered to team members on Dec. 26 within the approved 12-hour post-refrigeration window, unfortunately, most of it had to be discarded due to the temperature storage requirements necessary to maintain its viability."

The health care system said 17,000 team members have been vaccinated in the last 12 days.

"We are clearly disappointed and regret this happened," a spokesperson said.
